Why refer first responders

30+ years in emergency services

First responders often present differently in therapy.

You may notice:

  • Limited engagement despite attendance

  • Minimal discussion of job-related experiences

  • Use of humor, deflection, or intellectualization

  • A sense that something important is not being accessed

This is often less about resistance and more about cultural mismatch.

Working with a provider familiar with the culture can significantly improve engagement and retention.

When to refer

When it may be helpful to refer:

  • A first responder client isn’t engaging in therapy

  • They minimize or avoid job-related experiences

  • You suspect moral injury of cumulative trauma

  • Progress feels limited despite effort

Consider referring when a client:

  • Is a firefighter, paramedic, or law enforcement officer

  • Has difficulty connecting in therapy

  • Minimizes or avoids job-related experiences

  • Presents with cumulative trauma or burnout

  • Is experiencing moral injury (guilt, shame, value conflict)

  • Has had limited progress in prior therapy
